As of January 2021, a sanitation fee will be added to your normal Kahramaa water consumption bill and will be equal to 20% of the monthly water consumption, according to a Tweet by Kahramaa:

The sanitation fee will become a regular from your February 2021 Kahramaa bill, and is aimed towards improving the efficiency and quality of infrastructure services provided by the government, to rationalise water consumption and preserve water resources in Qatar, based on a report on the Ashghal website.
Qatari citizens are exempt from this fee, but non-Qatari residents and establishments will have to pay the sanitation fee. For example:
"If the value of the water bill is QR 300, then the value of wastewater charges added to the bill will be QR 60 only." (Ashghal)
